San Juan County Health & Community Services is proud to release the 2024 Community Health Assessment Report. San Juan County (SJC) partnered with our community to understand health needs, gaps, and existing community strengths. There was focused outreach to include the voices and experiences of our community partner agencies and of our Latinx and Spanish-speaking community.

The information collected during this process identified the top needs our community is facing around health and wellness and will help us plan the actions and services needed to improve health in our community. The last CHA of San Juan County was completed nearly 15 years ago. It was vital for us to collect updated information, especially after the COVID-19 response, to get a full picture of the health and wellness of island residents.

A full version of the CHA is available online here. There are also physical copies of the CHA distributed across the county for reading. To find a location with a physical copy near you, please see the list below or call our front office at 360-378-4474 and we will be glad to assist.

Health & Community Services staff will work with community partners to create a Community Health Improvement Plan (CHIP). The CHIP will identify priority areas of improvement, ways to evaluate our progress, and set a path towards creating a healthier community for all in San Juan County.

The Community Health Survey will remain open and is available to take online in English and Spanish. Even though they will not be reflected in this year’s CHA, your responses are valuable and will be considered as our team moves into the next stages of this project.

    SAN JUAN COUNTY, WA. November 28, 2023 – San Juan County Health & Community Services wants to hear from you! The Community Health Survey is collecting responses through Friday, Dec. 1 to include in the Community Health Assessment Report, which will be released in early 2024. If you haven’t taken the quick survey yet, be sure to do so!

    The anonymous survey takes only eight to ten minutes and will help HCS better understand how all islanders are doing. The results of the survey will identify areas of improvement and set a path towards creating a healthier community for all. There are questions about quality of life, access to health care, how people get health-related information, and what are some of the barriers to good health in our county. The more people we hear from, the more complete our picture will be of how our San Juan County community members are doing.

    You can take the survey online in English or Spanish. If you are having trouble accessing the survey online, please call the HCS front office at 360-378-4474 to ask to take the survey in another format.

    The survey results collected by Friday, Dec. 1 will be compiled and analyzed along with other data about our community. The results of the surveys will be published as a final report and will be available to our community in early 2024.

    El lunes 31 de julio de 2023, departamento de salud y servicios comunitarios del condado de San Juan; (HCS) lanzó su encuesta en todo el condado como parte del proceso de evaluación de salud comunitaria que está puesta en marcha. ¡Esta encuesta está abierta a todos los isleños y todos están invitados a hacer oír su voz! la encuesta es anónima y solo toma de ocho a diez minutos completarlo.

    Esta encuesta de 42 preguntas ayudará a obtener una imagen de cómo les está yendo a todos los residentes del condado de San Juan. Hay preguntas sobre la calidad de vida, el acceso a la atención médica, cómo las personas acceden a la información relacionada con la salud y cuales son algunas de las barreras para la buena salud en nuestro condado.     ¿Qué es una evaluación de salud comunitaria?

    La salud pública utiliza una evaluación de salud comunitaria como herramienta para observar la salud general de una comunidad. Este es un proceso que analiza las formas en que muchas organizaciones, escuelas, agencias y políticas en una comunidad contribuyen a la salud de su comunidad.     On Monday, July 31, 2023, San Juan County Health & Community Services (HCS) launched its county-wide survey as a part of the Community Health Assessment process which is underway. This survey is open to all islanders, and everyone is invited to make their voice heard! The survey is anonymous and only takes eight to ten minutes to complete.

    This 42-question survey will help paint the picture of how all San Juan County residents are doing. There are questions about quality of life, access to health care, how people access health-related information, and what are some of the barriers to good health in our county. The more people we hear from, the more complete our picture will be of how San Juan County residents are doing.

    The survey will be open until mid-November 2023 and is available online. If you are having trouble accessing the survey online, please call the HCS front office at 360-378-4474 to ask for assistance in taking the survey in another format.

    What Is a Community Health Assessment?

    Public health uses a Community Health Assessment as one tool to look at the overall health of a community. This is a process that looks at the ways many organizations, schools, agencies and policies in a community contribute to the health of their community. The information collected during this process can identify the top issues a community is facing around health and wellness and help plan the actions and services needed to improve health in our community.

    The survey results collected by Nov. 15 will be compiled and analyzed along with other data about our community. The results of the surveys will be published as a final report and will be available to our community in early 2024. HCS staff will work closely with our partners around the islands to use the findings to create a Community Health Improvement Plan. The Community Health Improvement Plan will identify areas of improvement and set a path towards creating a healthier community for all in San Juan County.

    The survey is available online through the San Juan County Engage platform. The survey is also available in Spanish. For those who cannot access the survey online, but would still like to participate, please call the HCS mainline at 360-378-4474 for assistance.
